
As part of our ongoing effort to deliver superior health care services to our valued patients, we are excited in introduce “Connected Care”, a new Medicare program for qualified Medicare Patients, aimed at filling any healthcare gaps, allowing your Primary Physician to better coordinate your care and progress between specialist, testing centers, and hospitals so everyone is working together.
Medicare has introduced this Connected Care program for Primary Care practices to implement across the country. The program was designed specifically to help Physicians reach out to their patients beyond the office, to ultimately ensure we regularly and proactively foster the most effective balance between the physician’s plan and the patient’s daily lifestyle.
Our Connected Care Department, which consists of highly trained Clinical Staff, will be contacting our qualified patients based on Medicare’s CMS guidelines.
Your dedicated Personal Care Coordinator will follow up after your visits and provide monthly check-in’s to keep you healthier. They will help to ensure your medications are not only working as expected (without side effects) but refilled in a timely basis. To reduce your phone waiting time, they will also assist setting appointments for specialist or diagnostics tests. Most importantly they will be updating this critical information in your chart and informing your doctor every month, of your specific progress.
What does CCM mean for you?
- CCM means having a continuous relationship with a dedicated health care professional who knows you and your history, gives you personal attention, and help you make the best choices for your health. Connecting to a dedicated health care professional means you can better manage your care and spend more time focusing on your health.
- CCM means you and your loved ones have the assistance you need to manage your chronic conditions so you can spend more time doing the things you enjoy. Your dedicated health professional will help you keep track of your medical history, medications, and all the doctors you see. You’ll receive a comprehensive care plan that outlines your treatment plan and goals.
- CCM means having help on call 24/7 to address urgent needs from the comfort of your home. CCM includes 24-hour-a-day, 7-day-a-week (24/7) access to health care professionals for urgent needs, including a means to make contact regardless of the time of day or day of week.
